Speech Language Pathologist – Home Health Visits

About the role

BAYADA Home Health Care is expanding its home health office in Fairfax, VA, and seeks an experienced Speech Language Pathologist for per‑diem home visits. You will provide rehabilitative speech, language, communication, and swallowing care to adult and geriatric patients in their own homes.

Key responsibilities

• Make home visits to clients across designated Fairfax territories.
• Conduct comprehensive evaluations, diagnose speech and swallowing disorders, and develop individualized treatment plans.
• Deliver therapeutic interventions and continuously revise care plans based on progress and physician orders.
• Educate patients, families, and caregivers on strategies to promote health, safety, and independent living.
• Document all observations, interventions, and evaluations using a tablet‑based electronic medical record system.

Required profile

• Current Virginia license as a Speech Language Pathologist.
• Certificate of Clinical Competence (CCC‑SLP) from ASHA or eligibility to sit for the ASHA exam.
• Ability to meet certification requirements through education and supervised experience.
• Prior home‑care experience is a plus but not mandatory.

Required skills

• Solid computer proficiency.
• Experience with electronic medical records (EMR) systems.
