Outpatient Occupational Therapist

About the role

BAYADA Home Health Care is seeking an experienced Occupational Therapist to provide outpatient services for adult and geriatric patients in senior living communities throughout Fairfax, VA. The role offers flexibility to work full‑time, part‑time, or per‑diem in a setting that blends home‑based care with outpatient resources.

Key responsibilities

• Collaborate with home health therapy partners to transition care from home health to outpatient services, ensuring client‑centered goals are met.
• Educate clients, families, and caregivers on care plans to maximize functional independence.
• Conduct outpatient therapy evaluations and develop individualized plans of care for residents.
• Provide evaluations, treatments, and discharge planning using the outpatient EMR platform.

Required profile

• Current Virginia Occupational Therapist license.
• Minimum one year of recent OT experience; prior home care experience preferred.
• Graduation from an accredited OT program.
• Ability to work independently, strong interpersonal and problem‑solving skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills for documentation and treatment planning.
