Service Coordination Specialist (Entry Level)

About the role

The Service Coordination Specialist joins the Crisis Relief Team to coordinate care and discharge planning for adults in intensive mental health and psychiatric services. This entry‑level position works overnight shifts alongside an interdisciplinary treatment team.

Key responsibilities

• Educate individuals on consents, treatment authorizations, and disclosures.
• Participate in interdisciplinary team meetings and coordinate external appointments and resources.
• Conduct discharge assessments, identify service gaps, and make referrals to ensure continuity of care.
• Maintain accurate clinic records, obtain required signatures, and document all interventions and coordination activities.

Required profile

• Washington State Registered Agency Affiliated Counselor (RAAC) credential required or eligibility to obtain.
• Pass Washington State Patrol background check at hire and annually thereafter.
• Effective verbal and written communication skills.
• Ability to work dynamically within a multi‑disciplinary team and maintain professional relationships.

• Physical intervention techniques for aggressive or self‑injurious behavior
