Senior LTSS Service Care Manager (RN)

About the role

The Senior LTSS Service Care Manager (RN) provides comprehensive care management for high‑acuity, long‑term services and supports (LTSS) members. Working in the field, you will assess needs, develop personalized care plans, and coordinate services across the continuum of care to ensure high‑quality, cost‑effective outcomes.

Key responsibilities

• Conduct UAS assessments for MLTC members in Schuyler, Steuben, Livingston and Alleghany counties and develop individualized service plans.
• Evaluate complex or high‑risk members, recommend optimal care pathways, and continuously monitor progress toward plan goals.
• Coordinate with members, families, caregivers, and providers to secure necessary medical, social, and community resources.
• Track member status, identify clinical changes, and adjust care plans for higher levels of care or waiver eligibility as needed.
• Document all care‑management activities to meet state, federal, and clinical compliance standards.
• Educate members and families on disease processes, care gaps, benefits, and available community resources.
• Serve as liaison and advocate between members, physicians, and facilities, ensuring person‑centered care.
• Precept and coach new clinical hires, guiding them through onboarding and competency checkpoints.

Required profile

• Graduate from an accredited nursing school with a Bachelor’s degree in Nursing (preferred) and 4–6 years of related experience.
• Current NY RN licensure (or compact state licensure) – NP licensure also acceptable.
• Experience conducting assessments and managing care for high‑acuity, long‑term care populations.
• Strong knowledge of state and federal regulations governing LTSS and Managed Care Organizations.
