Senior Integrated Clinic Pharmacist

About the role

We are looking for a Senior Integrated Clinic Pharmacist to join our primary‑care organization. This remote position reports to the Associate Director Integrated Clinic Pharmacist and integrates pharmacy expertise into physician practices to improve patient outcomes.

Key responsibilities

• Evaluate pharmacotherapy for high‑risk patients with chronic conditions such as diabetes, hypertension, heart failure, and COPD using evidence‑based medicine.
• Execute clinical protocols within organizational and state regulatory guidelines.
• Optimize drug coverage benefits, including preferred formulary products and lower‑cost alternatives.
• Collaborate with physicians, nurses, and other healthcare professionals to adjust treatment plans.
• Participate in multidisciplinary high‑risk rounds and provide patient counseling.
• Conduct disease‑state education and post‑discharge medication reconciliation.
• Document all activities in the clinic EMR and support CMS Stars and HEDIS quality measures.
• Lead in‑service education for staff and stay current with pharmacotherapy advancements.

Required profile

• Active, unrestricted pharmacist license in Florida (willingness to obtain additional state licenses).
• Pharm.D. or other advanced pharmacy degree.
• Minimum five years of practicing pharmacist experience.
• Experience working in a team environment supporting common goals.
