RN Case Manager - Hospice Care

About the role

Amedisys, part of the Optum family of businesses, offers home health, hospice and community-based services. As an RN Case Manager, you will coordinate compassionate, individualized hospice care by assessing patients, working with the interdisciplinary care team and developing care plans that meet each patient's needs. The position is based in Bluefield, West Virginia.

Key responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care
• Make referrals to other hospice disciplines as needed, or document the rationale for not doing so
• Develop, prepare and maintain accurate, timely patient care progress records in line with care center policies
• Record pain and symptom management changes and outcomes as appropriate
• Teach patients, families and caregivers appropriate care techniques
• Communicate regularly with the clinical manager and care team
• Perform on-call duties and services to patients and families as assigned

Required profile

• Current RN license for the state(s) of assignment
• 1+ year of nursing experience
• Valid driver's license, reliable transportation and liability insurance
• Current CPR certification

• Salary of $67,660 to $101,490 per year for full-time employment, depending on factors such as local market, education and experience
