RN Care Coordinator – Urology

About the role

The RN Care Coordinator – Urology works on Cleveland Clinic’s Main Campus to provide longitudinal care coordination and disease management for high‑risk urology patients. The position focuses on patient outreach, education, and transition support to improve outcomes and reduce unnecessary emergency visits.

Key responsibilities

• Collaborate with a multidisciplinary team to develop care plans, set goals, and monitor patient progress.
• Conduct comprehensive clinical assessments covering medical, behavioral, pharmacy, social, and end‑of‑life needs.
• Provide education, coaching, and compliance monitoring for patients and families.
• Serve as a liaison and advocate, facilitating transitions of care and connecting patients to community resources.

Required profile

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• 3–5 years of nursing experience.
• Basic Life Support (BLS) certification through AHA or Red Cross.
• For oncology‑related work, obtain a cancer‑specific certification or maintain qualifying education within the facility’s accreditation cycle.
