RN Care Coordinator - Hepatology

About the role

The RN Care Coordinator in the Hepatology Unit will work with a multidisciplinary team to provide longitudinal disease management and care coordination for high‑risk patients, aiming to improve outcomes and reduce readmissions.

Key responsibilities

• Collaborate with multidisciplinary caregivers to develop goals, plan interventions, and maximize patient outcomes.
• Identify patients needing coordination, conduct targeted outreach, and manage their care plans.
• Utilize assessment tools and technological resources (registries, patient lists, care team tab) to manage patient populations.
• Perform comprehensive clinical assessments covering medical, behavioral, pharmacy, social, and end‑of‑life needs.
• Educate and coach patients and families on self‑management, monitor compliance, and update care plans.
• Serve as liaison and advocate, facilitating transitions of care and connecting patients to community resources.

Required profile

• Graduate from an accredited nursing program and hold a current RN license.
• Basic Life Support (BLS) certification.
• Three to five years of nursing experience, preferably in care coordination.
• Preferred: Bachelor of Science in Nursing (BSN) and specialty certification.

Required skills

• Basic Life Support (BLS) certification.
• Proficiency in using care coordination technologies such as registries and patient list tools.
• Strong clinical assessment and risk‑assessment abilities.
