PRN RN – Rapid Response

About the role

The PRN RN – Rapid Response will join the Cleveland Clinic West Palm Beach team to provide patient‑first care for high‑risk patients. This regular PRN position works days with on‑call and holiday requirements, focusing on rapid response and care coordination.

Key responsibilities

• Collaborate with a multidisciplinary team to develop goals, plan interventions, and maximize patient outcomes.
• Conduct comprehensive clinical assessments, including disease‑specific, age‑specific, medical, behavioral, pharmacy, social and end‑of‑life needs.
• Identify patients needing ongoing coordination, perform targeted outreach, and manage care transitions across settings.
• Utilize assessment tools, registries, patient lists, and other technological resources to manage populations.
• Educate patients and families on self‑management, specialty care pathways, and community resources.

Required profile

• Graduate from an accredited nursing program with current RN licensure in Florida.
• Minimum one year recent RN experience.
• Basic Life Support (BLS) certification.
• Preferred: BSN, specialty certification, ACLS, outpatient or ED experience, care‑coordination experience.

Required skills

• Basic Life Support (BLS) certification.
• Advanced Cardiac Life Support (ACLS) certification (preferred).
• Proficiency with clinical assessment and risk‑assessment tools.
