Remote Nurse Practitioner – Telehealth Urgent Care

About the role

We are seeking a compassionate Nurse Practitioner to join our fast‑growing telehealth team. In this fully remote position you will provide urgent, non‑emergent clinical support to Medicare beneficiaries enrolled in our Chronic Care Management program, helping keep members out of the emergency department.

Key responsibilities

• Conduct telehealth evaluations for members with urgent clinical concerns such as poorly controlled hypertension or medication issues.
• Review medical histories, assess symptoms, and determine appropriate interventions or escalations.
• Document all encounters accurately in the electronic medical record.
• Provide evidence‑based treatment recommendations and patient education on disease management, lifestyle, and medication adherence.
• Collaborate with nurses, community health workers, primary care providers, and care coordinators.
• Participate in case reviews and quality‑improvement initiatives to reduce avoidable ED visits and hospitalizations.

Required profile

• Active, unrestricted Nurse Practitioner license in the United States.
• National certification appropriate to your specialty.
• Minimum of 3 years of clinical practice, preferably in primary care, internal medicine, family medicine, geriatrics, urgent care, or chronic disease management.

Required skills

• Proficiency with electronic medical record (EMR) systems.
