Remote Geriatric Physician (MD/DO) – 50-State Coverage

About the role

Sailor Health is seeking a board‑certified MD or DO to provide comprehensive telehealth care for older adults with chronic and complex conditions across all 50 states. You will act as the clinical gateway, partnering with healthcare advocates to create and oversee holistic care plans that integrate medical and social needs.

Key responsibilities

• Conduct thorough telehealth assessments for geriatric patients, identifying clinical risks, medication gaps, and recent utilization patterns.
• Collaborate with therapists, care navigators, and advocates to develop integrated care plans that address both physical health and social determinants.
• Monitor and adjust patient care plans, ensuring continuity and alignment with evolving health status.

Required profile

• MD or DO, licensed in at least 45 states (including FL, MD, PA, TX).
• Board certification in Internal Medicine, Family Medicine, or a related specialty.
• Minimum of 5 hours weekly availability and experience caring for older adults with chronic, complex conditions.
• Strong clinical judgment to synthesize incomplete information into clear, actionable plans.
