Rehabilitation Specialist

About the role

The Rehabilitation Specialist supports program members by gathering data, documenting goals and coordinating services in line with ISP regulations. This full‑time position works Monday‑Friday, 8:00 am‑4:00 pm, with some flexibility.

Key responsibilities

• Gather and document each member’s goals, objectives and progress.
• Coordinate services with case managers, parents/guardians and community partners.
• Screen, prioritize and track referrals.
• Implement behavior‑management strategies and investigate incidents.

Required profile

• High School diploma + 5 years related experience OR Associate’s degree + 3 years OR Bachelor’s degree + 1 year.
• Minimum 2 years case‑management experience.
• Valid driver’s license, good driving record and willingness to transport participants.
• Organized, detail‑oriented with strong time‑management skills.

Required skills

• Proficiency with Microsoft Word.
• Data‑entry, email and internet usage.
• Ability to use a personal or agency vehicle for participant transport.
