Weekend Registered Nurse – Full Time Home Health

About the role

Integrity Home Care + Hospice is seeking a full‑time Weekend Registered Nurse to provide skilled nursing and rehabilitative care to clients in the Springfield, MO area, enabling them to remain safely at home.

Key responsibilities

• Conduct comprehensive head‑to‑toe assessments and identify subtle changes in patient condition.
• Provide acute and post‑acute care including CHF, COPD, wound care, IV therapy, and post‑surgical care.
• Triage and manage urgent clinical situations independently in the home setting.
• Perform medication reconciliation, patient education, and safety monitoring.
• Document wound assessments, care, and other clinical information using point‑of‑care technology.

Required profile

• Current Missouri nursing license in good standing.
• Graduated from an accredited nursing program.
• At least one year of recent hands‑on experience with adult/geriatric patients in acute care, emergency, home health, hospice, ICU, med‑surg, sub‑acute or rehab settings.
• Ability to work independently, prioritize urgent visits, and escalate concerns when needed.

Required skills

• Acute and post‑acute care expertise (CHF, COPD, wound care, IV therapy, post‑surgical care).
• Medication reconciliation and patient teaching.
• Point‑of‑care documentation and technology proficiency.
