Rehabilitation Manager – Home Health (OT/PT) – Senior Living

About the role

BAYADA Home Health Care is seeking a Rehabilitation Manager for its Wilmington, MA Senior Living office. You will lead a team of physical, occupational, and speech‑language therapists, overseeing home‑based therapy services and ensuring high‑quality, outcomes‑focused care.

Key responsibilities

• Lead, mentor, and support a multidisciplinary therapy team, including potential nursing staff.
• Provide clinical and operational oversight for home‑based rehabilitation across Wilmington and surrounding areas.
• Coordinate care from referral through discharge, ensuring accurate documentation and compliance.
• Facilitate orientation, onboarding, competency development, and ongoing education for staff.
• Partner with the Director and Clinical Manager on staffing, scheduling, referrals, and office performance.

Required profile

• Current professional license in Physical Therapy or Occupational Therapy.
• Experience delivering rehabilitation services in home health or community‑based settings.
• Strong clinical judgment, organizational, communication, and problem‑solving abilities.
• Willingness to travel throughout Wilmington and surrounding communities as needed.

Required skills

• Proficiency with electronic documentation and communication systems.
