Registered Nurse – Youth Residential Program (Collbran, CO)

About the role

PsyPhyCare is seeking a Registered Nurse to provide on‑site medical and wellness services for a federally supported residential youth program in Collbran, Colorado. The role involves direct clinical care for young adults aged 16‑24, health education, and coordination with healthcare providers.

Key responsibilities

• Triaging urgent and emergent medical situations and providing first aid.
• Conducting clinical assessments, monitoring chronic conditions, and administering medications.
• Coordinating referrals, follow‑up care, and maintaining accurate medical documentation.
• Tracking laboratory results, immunizations, and ensuring regulatory compliance.
• Delivering individual and group health education sessions.
• Maintaining medical and prescription supply inventories and providing staff training on health topics.
• Collaborating with community healthcare partners and program leadership.

Required profile

• Current, unrestricted RN license in Colorado.
• Minimum two years of nursing experience in clinical, school‑based, residential, or community health settings.
• Experience working with adolescents or young adults preferred.
• Associate Degree in Nursing required; BSN preferred.
