Registered Nurse PRN Home Health

About the role

Amedisys, part of the Optum family of businesses, offers home health, hospice and community-based services. As a Registered Nurse PRN Home Health, you will work in Burlington, North Carolina, providing care to patients and families at home. The role involves assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care under the supervision of an RN or MD.

Key responsibilities

• Perform patient assessments and help the care team develop and implement a plan of care
• Make referrals to other disciplines or document the rationale for not doing so
• Teach patients and promote their health and independence through rehabilitation and self-care techniques
• Supervise LPNs and HHAs
• Complete documentation timely and accurately, and share patient progress with the clinical manager and care team
• Plan and deliver staff education, and perform on-call duties as assigned
• Take part in clinical development and continuing education programs

Required profile

• Current RN license for the state(s) of assignment
• At least 1 year of clinical experience as a Registered Nurse
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance

What we offer

• Pay of $71,677 to $107,516 annually, with per-visit point pay and an hourly rate for non-visit activities
