Registered Nurse – Patient Aligned Care Team (Outpatient)

About the role

The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) provides direct nursing care to veteran patients in an outpatient setting. The RN is accountable for safe, high‑quality nursing practice, appropriate task delegation, and ensuring patients receive timely evaluation and access to services.

Key responsibilities

• Deliver individualized nursing care and oversee delegation of tasks in accordance with nursing standards.
• Manage assigned patient panels, utilizing registries to track and coordinate care.
• Ensure appropriate clinical evaluation, follow‑up, and access to outpatient services for each patient.
• Maintain communication with supervising staff and adhere to rating and performance expectations.
• Document care activities accurately in the patient record.

Required profile

• Proficiency in written and spoken English as required by 38 U.S.C. 7403(f).
• Graduation from an ACEN‑ or CCNE‑accredited nursing program (or equivalent foreign accreditation with validated equivalency).
• Current, full, active, and unrestricted RN license in a U.S. state, territory, or the District of Columbia.
• Eligibility for Graduate Nurse Technician (GNT) appointment if licensure is pending, with a temporary appointment possible up to 120 days (extendable).
• Compliance with VA qualification standards effective 1/29/2024.
