Registered Nurse – Hospice (Full‑Time)

About the role

VitalCaring is seeking a compassionate Registered Nurse to provide hospice care in patients' homes. This full‑time, field‑based position focuses on delivering comfort, dignity, and clinical excellence during end‑of‑life journeys.

Key responsibilities

• Conduct comprehensive assessments of patients and families.
• Develop, implement, and adjust individualized comfort‑focused care plans.
• Provide skilled nursing care with an emphasis on pain and symptom management.
• Serve as case manager, coordinating care with physicians, social workers, chaplains, and aides.
• Educate and support families, helping them navigate complex decisions.
• Maintain accurate documentation and participate in interdisciplinary team meetings.

Required profile

• Active RN license (or compact eligibility) and graduation from an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment, compassionate approach, and ability to manage a caseload independently.
• Excellent communication skills for sensitive situations.
• Current CPR certification and a valid driver’s license with reliable transportation.

Required skills

• Basic EMR proficiency.
• Experience with Homecare Homebase (HCHB) preferred.
• CPR certification.
