Registered Nurse – Home Based Primary Care

About the role

The Registered Nurse will provide home‑based primary care to assigned veterans, delivering age‑appropriate, knowledge‑based nursing services. The role focuses on patient education, self‑management support, and ensuring satisfaction throughout the care continuum.

Key responsibilities

• Assess, plan, implement, and evaluate individualized care for each client.
• Coordinate care activities, emphasizing patient education and self‑management.
• Administer medications and procedures in accordance with established policies.
• Collaborate with interdisciplinary team members to influence care outcomes.
• Maintain accurate documentation and adhere to VA guidelines.

Required profile

• Proficiency in written and spoken English.
• Graduate of an ACEN‑ or CCNE‑accredited nursing program, or equivalent foreign credentials with recognized equivalency.
• Current, full, active, and unrestricted registration as a professional nurse in a U.S. state, territory, or the District of Columbia.
• Eligibility for a Graduate Nurse Technician (GNT) temporary appointment if licensure is pending.
