Registered Nurse Admissions – Home Health

About the role

BAYADA Home Health Care is seeking a full‑time Admissions Registered Nurse to provide skilled nursing and rehabilitative care to clients in Delaware County and parts of Philadelphia, enabling them to remain safely at home.

Key responsibilities

• Conduct comprehensive head‑to‑toe assessments and identify subtle changes in patient condition.
• Tri­age and manage urgent clinical situations independently in the home setting.
• Perform medication reconciliation, patient education, and safety monitoring.
• Provide wound assessment, care, and documentation.

Required profile

• Current Pennsylvania RN license in good standing.
• At least one year of recent hands‑on experience with adult/geriatric patients in acute, emergency, home health, hospice, ICU, med‑surg, sub‑acute or rehab settings.
• Previous home‑health experience is required.

Required skills

• Wound care assessment and treatment.
• IV therapy administration.
• Medication reconciliation.
