Registered Nurse – 1:1 Home Care (West Valley Area)

About the role

Join BAYADA’s Phoenix Skilled Nursing team to provide one‑on‑one home care for pediatric or geriatric clients in the West Valley area. You will deliver compassionate, high‑quality care directly in clients’ homes.

Key responsibilities

• Travel to a client’s home on a self‑selected schedule and provide dedicated care.
• Execute and document physicians’ orders, perform assessments, and monitor client conditions.
• Maintain accurate records of observations, interventions, and evaluations.
• Utilize nursing autonomy to perform skilled procedures safely in the home setting.

Required profile

• Graduation from an accredited nursing program (transcript or diploma required).
• Current Arizona RN license in good standing.
• At least one year of RN experience.
• Trach experience preferred but not mandatory.

Required skills

• Trach (tracheostomy) care experience.
• Proficiency in home‑health documentation and assessment tools.
