Rapid Response Registered Nurse

About the role

The Rapid Response RN works on weekends at the Cleveland Clinic West Palm Beach facility, providing patient‑first care throughout the care journey. The role involves direct interaction with patients and families, education, and coordination of high‑risk patient care.

Key responsibilities

• Collaborate with a multidisciplinary team to develop goals, plan interventions and maximize outcomes for high‑risk patients.
• Identify patients needing ongoing coordination, conduct targeted outreach, and manage disease‑specific care plans.
• Perform comprehensive clinical assessments covering medical, behavioral, pharmacy, social and end‑of‑life needs.
• Educate patients and families on self‑management, specialty or surgical conditions, and connect them to community resources.
• Serve as liaison and advocate, facilitating transitions of care and ensuring communication across settings.

Required profile

• Graduate of an accredited nursing program with current RN licensure in Florida.
• Three to five years of nursing experience, preferably in outpatient, emergency, or care‑coordination settings.
• Basic Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) preferred.
• BSN preferred; specialty certification and care‑coordination experience are a plus.

Required skills
