Program Manager – Habilitation Services

About the role

The Habilitation Manager will act as an internal case manager and advocate for clients in the Habilitation program, collaborating closely with the Clinical Manager and multidisciplinary teams.

Key responsibilities

• Collaborate with Clinical Manager, clients, families, and case managers to implement and evaluate individualized care plans.
• Conduct regular client visits to assess progress and ensure services align with Person Centered/Individual Support Plans.
• Supervise Habilitation Technicians in accordance with policy and state regulations.
• Participate in client-specific orientations and Person‑Centered Planning processes.

Required profile

• Bachelor’s degree required.
• Qualified Professional with at least 1 year of post‑degree experience serving the I/DD population.
• Prior supervisory experience preferred.
• Strong communication and interpersonal skills.
