PRN Pharmacist – Hospice Care (Arlington, TX)

About the role

OnePoint Patient Care is seeking a PRN Pharmacist to join its hospice pharmacy team in Arlington, TX. This non‑retail position focuses on delivering high‑quality medication management for hospice patients in a supportive, closed‑door environment.

Key responsibilities

• Receive and evaluate prescriptions for appropriateness, dosage, interactions, and route of administration.
• Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
• Maintain up‑to‑date patient medication profiles, including medical history and OTC drugs.
• Provide clinical recommendations to optimize medication therapy for hospice patients.
• Implement and enforce policies for safe procurement, storage, administration, and disposal of medications.
• Ensure compliance with state and federal regulations (CMS, DEA, FDA).
• Perform specialized tasks such as working in the Controlled Drugs room, supervising automated packaging machines, preparing IV medications, and supporting dispensing systems like Omnicell.

Required profile

• Bachelor’s degree in Pharmacy (Pharm D preferred).
• Current, valid pharmacist license in Texas.
• Knowledge of drug‑therapy guidelines, medication safety practices, and regulatory standards.
• Strong clinical judgment and effective written and verbal communication.

Required skills

• Proficiency with Electronic Medical Records (EMR) systems.
• Experience using pharmacy information systems.
