Private Duty Nursing Case Manager & RN Delegator

About the role

Journey Nursing Services seeks compassionate Registered Nurses to provide private duty nursing, case management, and RN delegation services across Washington State. The role blends direct patient care with clinical leadership and care coordination.

Key responsibilities

• Manage a caseload of private duty nursing clients, completing comprehensive assessments and developing individualized Plans of Care.
• Provide RN delegation services, including initial and renewal assessments, supervisory visits, and caregiver education.
• Deliver skilled nursing interventions for medically complex patients (e.g., tracheostomy, ventilator, TPN, enteral feeding).
• Coordinate care with physicians, therapists, pharmacies, and interdisciplinary team members.
• Maintain accurate electronic medical record documentation and participate in quality improvement initiatives.

Required profile

• Current Washington State RN license in good standing.
• Current BLS certification.
• Minimum one year of nursing experience.
• Strong assessment, clinical judgment, communication, and teaching abilities.
• Valid driver’s license, reliable transportation, and willingness to travel within the service area.
