Primary Care Physician – Tyler, TX

About the role

Oak Street Health is seeking a Primary Care Physician to join its Tyler, TX clinic. The physician will deliver equitable, value‑based care to Medicare patients, focusing on quality outcomes rather than volume.

Key responsibilities

• Assess, diagnose, and treat patients in the clinic.
• Prescribe and manage pharmaceutical treatments.
• Maintain accurate patient records using Canopy and Greenway EMR systems.
• Collaborate with regional leadership to achieve health quality goals.
• Work with practice managers to direct the care team.
• Participate in additional duties as assigned.

Required profile

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree.
• Board certified or board‑eligible in Internal Medicine or Family Medicine.
• Active, unrestricted state medical license and DEA license.
• U.S. work authorization.
• Motivation to serve older adult populations.

Required skills

• Proficiency with Canopy EMR.
• Proficiency with Greenway EMR.
