Primary Care Physician

About the role

ChenMed is seeking dedicated Primary Care Physicians to join its mission-driven, physician‑led organization. You will manage a panel of up to 450 senior patients, providing comprehensive, value‑based care that emphasizes health coaching, outcome accountability, and cost‑effectiveness.

Key responsibilities

• Deliver high‑quality clinical care to a defined patient panel, focusing on preventive health and chronic disease management.
• Act as a health coach, helping patients set and achieve short‑ and long‑term health goals.
• Track and improve performance metrics such as admissions per thousand, patient experience (CGCAHPS), clinical gap closures, and medical cost measures.
• Collaborate with care teams, managers, and medical directors to ensure compliance with guidelines and participation in risk‑ and quality‑management programs.
• Champion a positive culture of love, accountability, and passion within the medical center.

Required profile

• Board‑certified or eligible in Family Medicine, Internal Medicine, or related primary care specialty.
• Strong commitment to outcomes‑focused, value‑based care for senior populations.
• Ability to work both independently and as part of an interdisciplinary team.
• Excellent communication skills to build trusting relationships with patients.
