Population Health Manager – Northwest Arkansas

About the role

The University of Arkansas for Medical Sciences is seeking a Regional Population Health Manager to lead population health and value‑based care initiatives across its Primary Care Population Health Service Line in Northwest Arkansas. This role will support clinics in Fort Smith, Fayetteville, and Springdale, driving quality improvement and operational success.

Key responsibilities

• Collaborate with clinic leadership—including access, nursing, physician, and administrative teams—to meet quality and performance targets.
• Design and implement transformative projects that improve patient outcomes and standardize patient‑centered care.
• Lead and mentor Population Health Improvement Assistants to support clinic‑level initiatives.
• Report to the Quality Director for Population Health and Value‑Based Care, providing updates and aligning strategies.

Required profile

• Bachelor’s degree in a health‑related field (Master’s or BSN preferred).
• At least three years of experience in quality improvement, program or project management; five years in clinical quality improvement is a plus.
• Strong written and verbal communication skills to engage diverse stakeholders.
• Ability to travel frequently between Fort Smith, Fayetteville, and Springdale sites.

Required skills

• Proficiency in Microsoft Excel, Word, and PowerPoint.
• Experience with Electronic Health Record systems, preferably EPIC.
• Knowledge of HEDIS measures, risk adjustment (HCC), and quality improvement methodologies.
