Physical Therapist – Weekend SOC Home Health Visits

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist to provide weekend Start of Care (SOC) home health visits on a per‑diem basis. The role supports adult and geriatric clients throughout Howard County, MD, delivering rehabilitative care that enables patients to remain safely at home.

Key responsibilities

• Conduct home visits in designated territories, performing diagnostic tests, mobility assessments, strength and balance evaluations, and safety checks.
• Develop and implement individualized care plans that include therapeutic exercises, gait training, wound‑prevention interventions, and ongoing plan revisions in collaboration with nursing staff.
• Educate patients, families, and caregivers on rehabilitative activities and document all observations and interventions using a tablet‑based electronic medical record system.

Required profile

• Current Maryland Physical Therapist license.
• Minimum one year of recent PT experience, preferably with HH/Oasis documentation.
• Graduation from an APTA‑accredited PT program.
• Ability to work independently, manage time effectively, and possess strong interpersonal and problem‑solving skills.

Required skills

• Proficiency with HH/Oasis home health software.
• Experience using electronic medical records (EMR) on tablet devices.
