Physical Therapist – New Graduate Home Health Position

About the role

Join BAYADA Home Health Care as a new graduate Physical Therapist and provide one‑on‑one care to adult and geriatric clients in their homes on the Big Island. You will receive structured orientation, mentorship, and clinical support to transition confidently into independent practice.

Key responsibilities

• Evaluate and assess patients' functional status and mobility.
• Develop individualized treatment plans to maximize independence and safety.
• Provide skilled therapeutic interventions in patients' homes.
• Educate patients, families, and caregivers on exercises, mobility, fall prevention, and home safety.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
• Complete timely electronic documentation while maintaining high standards of patient care.

Required profile

• Graduate of an accredited Doctor of Physical Therapy (DPT) program.
• Licensed or license‑eligible Physical Therapist in the state of practice.
• New graduates are strongly encouraged to apply.
• Excellent communication and interpersonal skills.
• Valid driver’s license and reliable transportation.
