Physical Therapist – Home Health Weekends

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist for part‑time weekend home‑health visits in the Asheboro area. You will provide rehabilitative care to adult and geriatric clients throughout Randolph County, North Carolina.

Key responsibilities

• Conduct home visits in designated territories and perform diagnostic tests such as range‑of‑motion, strength, balance, and safety assessments.
• Develop, implement, and continuously revise individualized care plans that include therapeutic exercises, gait training, and wound‑care interventions.
• Educate clients and families on rehabilitative activities to promote independence and safety.
• Document all observations, interventions, and evaluations using a tablet‑based electronic medical record system.

Required profile

• Current North Carolina Physical Therapist license.
• At least one year of recent PT experience.
• Graduation from an APTA‑approved program.
• Ability to work independently, manage time effectively, and possess strong interpersonal and problem‑solving skills.

• Proficiency with electronic medical records (EMR) systems.
