Physical Therapist – Home Health Visits

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist for per‑diem home health visits in Scotland, Richmond, and Robeson counties. The role supports adult and geriatric clients, delivering rehabilitative care that enables them to remain safely at home.

Key responsibilities

• Conduct home visits within designated geographic territories and perform comprehensive diagnostic assessments (mobility, strength, balance, safety).
• Develop, implement, and continuously revise individualized care plans that include therapeutic exercises, gait training, and wound‑care interventions.
• Educate clients and families on rehabilitative activities and document all observations and interventions using a tablet‑based EMR system.

Required profile

• Current North Carolina Physical Therapist license.
• Minimum one year of recent clinical experience as a licensed PT.
• Graduation from an APTA‑approved PT program.
• Ability to work independently, manage time effectively, and collaborate within a multidisciplinary team.

Required skills

• Proficiency with electronic medical record (EMR) systems.
• Strong computer skills for tablet‑based documentation.
