Physical Therapist - Home Health

About the role

Arlington Assured Home Health, part of LHC Group and the Optum family, provides home health, hospice and community-based care. As a Physical Therapist in Home Health, you will assess patients' functional status, mobility, endurance, stamina, pain and other medical conditions, help set a treatment plan, deliver interventions and track progress.

Key responsibilities

• Evaluate patients' functional status and physical therapy needs, and assist the physician in developing the plan of care
• Provide services within the scope of practice allowed by state law, in line with the plan of care and with the health care team
• Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician
• Treat patients to relieve pain, restore function and maintain maximum functioning
• Instruct the patient, family and caregivers about the plan of care, functional limitations and progress
• Visit patients according to the plan of treatment, write a progress note for each visit and submit them per LHC policy
• Report evaluation outcomes, goals and the anticipated frequency of care

Required profile

• Current Physical Therapy licensure in the state of Washington
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Preferred: current CPR certification or ability to obtain it within 90 days of hire

What we offer

• Total cash target pay of $110,000 - $157,259 for full-time work (40 weekly hours), made up of per-visit point pay and hourly pay for non-visit activities
