MSW Social Worker - Home Health

About the role

Arlington Assured Home Health, part of LHC Group and the Optum family, provides home health, hospice and community-based services. It is hiring a Master of Social Work (MSW) social worker who provides medical social services under the direction of a physician and an interdisciplinary group. The role helps patients understand the social and emotional factors affecting their health and develop coping mechanisms.

Key responsibilities

• Help patients, family members, physicians and the health care team understand personal, emotional, environmental and social factors related to the patient's health status, as needed
• Contribute to the development of a comprehensive, integrated Plan of Care for patients on a daily basis
• Instruct health care team members about community resources available to patients, as needed

Required profile

• Master's degree from a Council on Social Work Education (CSWE)-accredited school of social work
• Current driver's license, vehicle insurance and access to a reliable vehicle or public transportation
• Washington state requirement: if not licensed as a social worker, an Agency Affiliated Counselor credential is required
• Preferred: 1+ years of social work experience in a health care setting, current CPR certification (or ability to obtain within 90 days of hire), and a Washington state social worker license

What we offer

• Salary of $100,000 to $108,000 per year for full-time employment, depending on factors such as local labor market, education, experience and certifications
