Physical Therapist – Home Health (Full‑Time)

About the role

BAYADA Home Health Care is seeking a compassionate Physical Therapist to join its Ocean County Senior Living team. You will provide one‑on‑one therapy services to adult and geriatric clients in assisted‑living, independent‑living and other senior housing settings across Ocean County, NJ.

Key responsibilities

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
• Deliver evidence‑based interventions such as therapeutic exercise, gait and balance training, fall prevention and functional mobility programs.
• Collaborate with nurses, physicians, caregivers and other professionals to coordinate patient‑centered care.
• Document assessments, progress notes and treatment outcomes accurately using electronic medical records.

Required profile

• Current New Jersey Physical Therapist license.
• Minimum one year of recent clinical experience as a Physical Therapist.
• Graduated from an APTA‑accredited Physical Therapy program.
• Ability to work independently, manage time effectively and communicate clearly with patients and team members.

Required skills

• Proficiency with electronic medical records and point‑of‑care technology.
• Strong clinical assessment and reasoning abilities.
