Physical Therapist – Home Health Admissions

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist to perform home health admission visits for clients in senior living settings across Essex and parts of Middlesex County. This full‑time role focuses on Start of Care/Admission visits and requires recent Medicare‑certified home health documentation experience.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, including range of motion, strength, balance, and safety assessments.
• Develop and implement individualized care plans with therapeutic exercises, gait training, and balance interventions.
• Educate clients, families, and caregivers on rehabilitative care and independent living strategies.
• Accurately document observations, interventions, and evaluations using a tablet‑based electronic medical record system.

Required profile

• Current MA Physical Therapist license.
• Minimum one year of recent PT work experience, preferably in Medicare‑certified home health.
• Graduation from an APTA‑approved PT program.
• Ability to work independently, manage time effectively, and possess strong interpersonal and problem‑solving skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills for documentation on tablet devices.
