Patient Care Coordinator / Swing Bed Coordinator

About the role

The Patient Care Coordinator / Swing Bed Coordinator serves as the primary liaison between patients, families, and healthcare providers at Citizens Health. This full‑time position ensures seamless communication, compliance with regulations, and coordination of swing‑bed services to enhance patient outcomes.

Key responsibilities

• Coordinate admissions, discharges, and assessments for the swing‑bed program.
• Verify insurance coverage, swing‑bed benefit days, and obtain necessary authorizations.
• Arrange transport, durable medical equipment, home health, and rehabilitation services.
• Monitor program utilization, quality metrics, and regulatory compliance.
• Facilitate communication among physicians, nurses, social workers, and other staff.
• Educate referring facilities, community members, patients, and families about the swing‑bed program.

Required profile

• High School Diploma (college degree in healthcare preferred).
• Prior hospital experience with swing‑bed, discharge planning, or case management.
• Knowledge of Medicare and other regulatory requirements.
• Strong interpersonal and communication skills.

Required skills

• Proficiency with office productivity software (email, calendar, word processing, spreadsheets, presentation tools).
• Ability to maintain accurate patient records and documentation.
