Part‑Time Community Health Worker – Spanish & Somali

About the role

CHW Solutions is seeking two part‑time Community Health Workers to deliver home‑visiting services for families with preschool‑aged children in the Twin Cities metro area. One position requires Spanish fluency and the other Somali fluency, supporting culturally relevant health promotion.

Key responsibilities

• Conduct six monthly home visits (2‑3 visits per week) and 2‑3 weekly check‑ins via phone, Zoom, email or text.
• Use motivational interviewing and positive parenting techniques to help families establish healthy routines around meals, play, and sleep.
• Document visits in the CHW Solutions electronic health record and enter research data into the University of Minnesota REDCap database.
• Participate in bi‑weekly team meetings with university research staff and assist in refining health education practices.

Required profile

• Excellent communication skills in English and either Spanish or Somali.
• Valid driver’s license, reliable vehicle, and ability to travel up to 45 minutes one‑way for home visits.
• Availability for occasional evening work (until 7 PM) and completion of required research trainings.

Required skills

• Motivational Interviewing.
• Use of electronic health record systems and adherence to HIPAA.
• Experience with REDCap data entry.
