Outpatient Physical Therapist – Senior Living Visits

About the role

BAYADA Home Health Care is seeking an experienced Outpatient Physical Therapist to provide on‑site therapy services to residents of senior living communities throughout Anne Arundel County, Maryland. The role blends the personal touch of home‑based care with the resources of an outpatient clinic, allowing you to treat patients in their apartments or in a fully equipped therapy gym.

Key responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests, mobility assessments, strength and balance evaluations, and safety screenings.
• Develop, implement, and continuously revise individualized care plans, including manual therapy, gait training, and balance exercises.
• Educate clients, families, and caregivers on rehabilitative activities that promote independence and safety.
• Document all observations, interventions, and evaluations using a state‑of‑the‑art tablet EMR system.

Required profile

• Current Maryland Physical Therapist license.
• Graduation from an APTA‑approved PT program.
• Ability to work independently, manage time effectively, and solve problems.
• Strong interpersonal and organizational skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills for documentation on tablet devices.
