Outpatient Physical Therapist – Senior Living

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist to provide outpatient home‑health visits for adult and geriatric clients in the Pineville, NC area. You will join a multidisciplinary team delivering rehabilitative care that enables clients to remain safely at home.

Key responsibilities

• Make scheduled outpatient visits to clients within designated geographic territories.
• Perform diagnostic tests, range‑of‑motion measurements, balance and safety assessments, and develop individualized care plans.
• Implement manual therapeutic exercises, gait training, balance work, and other interventions, adjusting the plan as needed.
• Educate clients and families on rehabilitative activities that promote health, safety, and independence.
• Document all observations, interventions, and evaluations using a tablet‑based electronic medical record system.

Required profile

• Current North Carolina Physical Therapist license.
• Minimum one year of recent PT experience.
• Graduation from an APTA‑approved PT program.
• Ability to work independently, manage time effectively, and solve problems.

Required skills

• Proficiency with electronic medical record (EMR) systems.
• Strong computer skills for documentation and scheduling.
