Occupational Therapist – Home Health (Full‑time)

About the role

BAYADA Home Health Care is seeking an experienced Occupational Therapist to provide home‑health services to adult and geriatric patients in the Charlotte, Steele Creek, and Pineville areas. This full‑time position involves independent visits to clients’ homes, delivering rehabilitative care as part of a multidisciplinary team.

Key responsibilities

• Conduct home visits within designated geographic territories and perform comprehensive functional evaluations.
• Develop individualized intervention plans, assess home environments for hazards, and recommend equipment or adaptations.
• Collaborate with nursing staff on multidisciplinary care plans and continuously revise therapy plans based on patient progress.
• Educate patients, families, and caregivers on therapeutic activities and document all observations and interventions using a tablet‑based EMR system.

Required profile

• Current North Carolina Occupational Therapist license and at least one year of recent OT experience.
• Graduation from an accredited occupational therapy program.
• Ability to work independently, manage time effectively, and possess strong interpersonal and problem‑solving skills.
• Prior home‑care experience is preferred but not mandatory.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills for documentation and telehealth tools.
