Occupational Therapist – Senior Living Visits

About the role

BAYADA Home Health Care is seeking an experienced Occupational Therapist to provide on‑site visits for senior living communities in Summit County. This full‑time role supports adult and geriatric patients in independent and assisted living settings, helping them maintain independence and safety at home.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough functional evaluations, including muscle function, endurance, visual coordination, communication skills, ADL and IADL performance.
• Assess cognitive, psychological and motor aspects and develop individualized intervention plans.
• Evaluate home environments for hazards and recommend equipment or adaptations.
• Educate patients, families, and caregivers on rehabilitative care and safety.
• Accurately document observations and interventions using a tablet‑based electronic system.

Required profile

• Current Ohio Occupational Therapist license.
• Minimum one year of recent occupational therapy experience.
• Graduation from an accredited OT program (CAOT/AOTA).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving abilities.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills, including use of a touch‑pad tablet for documentation.
