Occupational Therapist – Home Visits

About the role

BAYADA Home Health Care is seeking a licensed Occupational Therapist to join its Oahu Visits team. You will provide in‑home assessments, develop individualized treatment plans, and help clients achieve greater independence while staying in their own homes.

Key responsibilities

• Conduct comprehensive functional evaluations covering muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological, and motor aspects and create personalized intervention plans.
• Evaluate the home environment for hazards and recommend equipment or adaptations.
• Collaborate with nursing and multidisciplinary teams to update care plans.
• Educate patients, families, and caregivers on therapeutic activities and safety.
• Document all observations, interventions, and outcomes using a tablet‑based system.

Required profile

• Current Occupational Therapist license in Hawaii.
• Willingness to travel within the assigned territory.
• Graduated from an accredited OT program (AOTA/AMA).
• Preferably at least 1 year of OT experience.
