Occupational Therapist – Home Health (Per Diem)

About the role

BAYADA Home Health Care is seeking an experienced Occupational Therapist for a per‑diem, home‑health position serving adult and geriatric patients in senior‑living communities throughout Lincolnton, North Carolina.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform comprehensive functional evaluations, including muscle function, endurance, visual coordination, communication skills, ADL and IADL performance.
• Assess cognitive, psychological and motor aspects and develop individualized intervention plans.
• Evaluate home environments for hazards and recommend equipment or adaptations.
• Continuously revise care plans and collaborate with nursing on multidisciplinary plans.
• Educate patients, families, and caregivers on rehabilitative care and safety.
• Document observations, interventions, and evaluations using a tablet‑based electronic system.

Required profile

• Current North Carolina Occupational Therapist license.
• At least one year of recent OT experience.
• Graduation from an accredited occupational therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving abilities.

Required skills

• Solid computer skills.
• Experience with electronic medical records (EMR) preferred.
