Occupational Therapist – Home Health (Part‑time)

About the role

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for part‑time home health visits in Forsyth County, North Carolina. You will work with adult and geriatric patients in their homes, providing rehabilitative care as part of a multidisciplinary team.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects and develop individualized intervention plans.
• Evaluate home environments for hazards and recommend equipment or adaptations.
• Continuously assess and revise care plans and collaborate with nursing staff.
• Educate patients, families, and caregivers on rehabilitative activities to promote safety and independence.
• Accurately document observations, interventions, and evaluations using a tablet‑based electronic system.

Required profile

• Current North Carolina Occupational Therapist license.
• Minimum one year of recent OT work experience.
• Graduation from an accredited OT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving abilities.

Required skills

• Solid computer skills.
• Experience with electronic medical records (EMR) preferred.
