Occupational Therapist – Home Health & Hospice

About the role

The Homecare Occupational Therapist (OT) at Phelps Health develops individualized care plans for patients receiving home health or hospice services, collaborating with interdisciplinary teams to achieve patient and physician goals.

Key responsibilities

• Evaluate patients for admission, conduct comprehensive assessments, and create age‑appropriate, goal‑driven care plans.
• Communicate progress and updates with physicians, caregivers, and the interdisciplinary team, including participation in IDT meetings.
• Supervise COTAs and homecare assistants, ensuring adherence to the plan of care.
• Document all services promptly, manage visit scheduling efficiently, and assist patients in obtaining necessary equipment.

Required profile

• Graduate of an accredited Occupational Therapy program.
• Minimum 1 year experience in a JCAHO‑accredited hospital and 1 year in an acute rehab facility; home‑care experience preferred.
• Current AOTA certification, Missouri OT license, CPR certification, and a valid driver’s license with automobile liability insurance.

Required skills

• Proficiency in patient assessment and care plan development.
• Ability to supervise and train support staff (COTA, homecare assistants).
