Nurse Practitioner – Street Medicine

About the role

AltaMed is seeking a dedicated Nurse Practitioner to join its Street Medicine program. You will provide comprehensive primary care to individuals experiencing homelessness and housing insecurity, delivering high‑quality, compassionate services in community settings.

Key responsibilities

• Conduct patient visits for new and established patients in street encampments, shelters, and supportive housing sites.
• Develop and coordinate individualized care plans with the street medicine team and external service providers.
• Make independent clinical decisions while consulting the Medical Director as needed.
• Document encounters and maintain accurate records in the electronic medical record system.
• Participate in outreach, health education, and community partnership activities.

Required profile

• Graduated from an accredited baccalaureate nursing program.
• Current California NP licensure and certification in Adult/Family Medicine.
• Experience diagnosing and managing common and complex medical conditions.
• Previous work with high‑risk or unhoused populations preferred.
• Bilingual in Spanish and English preferred.
• Valid BLS certification (American Heart Association or Red Cross).

Required skills

• Proficiency with computer‑based systems.
• Experience using Electronic Medical Records (EMR) platforms.
