Nurse Practitioner – Skilled Nursing (SNF)

About the role
Join Avir Health Group as a Nurse Practitioner in a Skilled Nursing Facility, providing clinical leadership for both short‑ and long‑term residents. You will collaborate with the care team to ensure high‑quality, patient‑centered care.
Key responsibilities
- Manage admissions and discharges for residents.
- Conduct monthly stability checks and evaluate changes in condition.
- Develop and oversee advanced care planning and comprehensive assessments.
- Prescribe appropriate treatments and medications.
Required profile
- Active Texas Advanced Practice Licensure (APN) and DEA license.
- Previous nursing or clinical experience.
- Strong communication and teamwork abilities.
- Master’s degree in Nursing.
