Nurse Practitioner – Primary & Palliative Care (Bucks County, PA)

About the role

Ennoble Care is seeking a Board‑Certified Nurse Practitioner to deliver primary and palliative care to homebound patients throughout Bucks County, Pennsylvania. You will work independently under physician supervision, providing compassionate, high‑quality care to a diverse patient population with chronic and complex conditions.

Key responsibilities

• Conduct comprehensive physical examinations and review medical histories.
• Develop and implement individualized care plans, including diagnosis, treatment, and follow‑up.
• Order and interpret diagnostic tests as medically necessary.
• Prescribe medications, supplies, and other therapeutic interventions.
• Communicate effectively with patients, families, caregivers, and internal clinical teams.
• Participate in quarterly medical staff meetings and maintain accurate documentation.

Required profile

• Board‑Certified Nurse Practitioner with a valid Pennsylvania license.
• 1–2 years of primary care experience, preferably with geriatric or hospice patients.
• Strong clinical judgment, problem‑solving ability, and independent work ethic.
• Positive demeanor and excellent interpersonal skills.
