Nurse Practitioner - Primary Care

About the role

Petaluma Health Center is recruiting a Nurse Practitioner to join its award-winning team and deliver primary medical care at the top of their licensure. The NP works as part of a team of health care professionals, following the center's standardized clinical procedures, policies, protocols and guidelines under the supervision of a physician supervisor.

Key responsibilities

• Take histories and physical examinations, and evaluate and treat patients within the usual and customary scope of licensure
• Order diagnostics and implement therapy according to scope of practice guidelines
• Consult the supervising physician when a case falls outside scope of practice or is unfamiliar
• Educate patients and communicate clearly during care
• Participate in Medical Staff and PA/NP meetings and other committees
• Maintain credentials, including the California Medical License, DEA licenses, CME hours, CPR certification and others
• Perform after-hours call, weekend clinics and assignments to outside locations within federal scope, such as school-based clinics
• Meet Joint Commission Ambulatory Care requirements and productivity targets

Required profile

• Nationally accredited Nurse Practitioner program completed
• Three to five years of primary care experience in a family practice setting
• Current CA RN license in good standing and current CA FNP or ANP certification
• Current BLS certification
• Ability to meet credentialing and privileging requirements
• Bilingual English and Spanish, written and verbal, preferred
• Experience in a NCQA PCMH Level 3 organization and a Joint Commission-accredited facility highly preferred
